Case Coordination- Link has two levels of case coordination to support the people served. Link Associates implements individually centered, family, and community focused services whereby persons served are assisted in identifying, securing, and sustaining resources necessary to live in the community. Our model prides itself on integrating case coordination with other services and ensuring comprehensive advocacy and supports necessary. Each Case Coordinator strives to ensure personalized services for the person served and their family.
• Case Management provides coordination of the Comprehensive Consumer Service Plan, crisis intervention, maintenance of person’s served records, monitoring of on-going services, comprehensive advocacy and support for person served eligible for Title XIX. The focus of our case managers is to educate, advocate for, and help you to obtain appropriate services. They will also coordinate and monitor the continued appropriateness of those services. The program is accredited by the State of Iowa.Any individual who qualifies for Iowa’s Intellectual Disability, Home and Community Based Services (HCBS) Waiver and who is not in a Medicaid-funded institution is eligible. Link Associates cannot provide your Case Management if you are currently enrolled with a Managed Care Organization (MCO), as the MCO will provide your Case Management.
• Program Management assists person’s served receiving day, employment and/or residential services from Link Associates, but have an external Case Manager, such as an MCO Case Manager, in the development of the Comprehensive Person’s served Service Plan, crisis intervention, maintenance of person’s served records, monitoring of on-going service, along with advocacy and correspondence with the external case manager.
