Request for
Services
Making decisions about who you will choose to support your
family member is an important step. Your personal
goals and your family desires are important to us. If you
are interested in learning more about the wide array of
services and supports we offer at Link Associates we would
love to visit with you and learn more about your needs.
To apply for services:
1.
Call Link Associates at 515-262-8888 and ask for the
Admissions Committee Chairperson. They will visit with
you about the interest of your family and the options we
have and a personal tour of the program(s) of interest will
be arranged for you.
2.
If you are interested in utilizing Link's services and an
opening is available in the preferred program or site,
application materials will be given to you for completion.
3.
If you are interested in utilizing services from Link
Associates and no opening is available, you may choose to
place your name on a referral list. When an opening becomes
available, the Admission Committee Chairperson will notify
you and give you the application materials at that time.
4.
Once all completed application materials are received, you
will be scheduled for an admissions meeting. These
Admissions Meetings are scheduled the third Wednesday of
every month. Following the meeting you will typically
be notified of the committee’s decision within 24 hours of
the meeting.
